ESSENTIAL FUNCTIONS (not limited to):
Meet employment needs of assigned caseload, including but not limited to: career exploration, job placement, career counseling, on and offsite coaching/support, and advocacy. Provide supports based on individual need to assist service participants to obtain and retain employment.
Assist and advise service participants on career development, including but not limited to: resume writing, interview preparation, and best career practices.
Train, support and monitor individuals at employment sites.
Provide individualized follow-along supports to assist service participants in maintaining employment. Write job support plans, incorporating input from the interdisciplinary team.
Transport and/or assist in securing appropriate transportation to secure employment.
Ensure each participant has a current and comprehensive electronic data base for all employment related information: assessments, work history, resume, job applications, interviews, positions, and employers.
Assist in development of assigned budget and monitor expenses to ensure budget compliance and fiscal accountability.
Remain current with systems changes in DDS and other funding agencies, particularly as the changes relate to reimbursement and waiver guidelines.
Comply with DDS and other funder reporting requirements.
Assist with coverage of Group Supported community based crews as needed.
Full time/flexible hours; 8:30am -4:30pm. May include some nights and weekends.
PREFERRED KNOWLEDGE AND SKILLS:
Knowledge of person-centered, outcome-based services for people with differing abilities, company policies and procedures, applicable regulatory standards and community resources
Skill in supervising and training staff, planning and organizing, problem solving
Effectively write and communicate in a courteous and professional manner
Attend to the physical and personal needs of adults with differing abilities, problem-solve effectively, maintain confidentiality, use adaptive and other equipment
Ability to read and understand budgets and financial information, work successfully independently and in a team environment, and work flexible hours in varying locations as needed
Bachelor’s degree preferred. High school diploma required; Minimum of 2 years of experience providing employment and/or transition supports to people with differing abilities. Must be able to provide support and services utilizing community based resources. Must be over 21 and possess a valid Connecticut driver’s license, have an acceptable driving history and be able to utilize your own vehicle for SARAH business as defined by SARAH Inc.’s policy.
REQUIRED PHYSICAL EFFORT: Must be able to sit, stand and walk at an assigned location(s) and work for extended periods of time; bend/stoop, kneel and balance frequently; climb, reach above shoulder level; push/pull, squat and crouch occasionally; with assistance, lift, carry and position individuals of various weight. Must be able to lift and carry up to 50 pounds occasionally. Must be able to drive multi-passenger vehicles for extended periods throughout the day.
